The hard truth is the lack of knowledge the blower has that made the piece. Many times it is a cadmium color that boils, but all glass will boil if heated too hot. A skilled blower will know how to use the colors so they do not boil.
Well it is simply adding a dot of glass, melting it round and then pressing in the dot. It is very time-consuming. See the Tentacle Rig here.
